Эректильная дисфункция (ЭД) и связанные с ней репродуктивные расстройства представляют собой значимую медицинскую и социальную проблему, особенно у пациентов с хронической болезнью почек (ХБП). У данной категории больных частота нарушений эректильной и репродуктивной функций существенно выше, чем в общей популяции, что негативно сказывается на их качестве жизни и психоэмоциональном состоянии.</p></sec><sec><title>Цель</title><p>Цель. Целью настоящего обзора является систематизация актуальных данных о распространенности, патогенетических механизмах и последствиях ЭД у пациентов с ХБП и у реципиентов трансплантированной почки, а также анализ влияния трансплантации почки (ТП) на восстановление половой и репродуктивной функции.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В обзор включены отечественные и зарубежные исследования преимущественно за последние 5 лет, посвященные оценке эректильной функции у пациентов с ХБП и после ТП. Особое внимание уделено использованию стандартизированных методов оценки, таких как Международный индекс эректильной функции (МИЭФ-5), а также анализу данных о гормональном фоне, психоэмоциональных и социальных аспектах.</p></sec><sec><title>Заключение</title><p>Заключение. ЭД – частое осложнение ХБП, обусловленное сосудистыми, гормональными и психоэмоциональными факторами. Эффективное лечение требует междисциплинарного подхода с учетом соматического и психологического состояния пациента. ТП способствует улучшению половой функции, однако у ряда пациентов ЭД сохраняется из-за иммуносупрессии и сопутствующих заболеваний. Необходимы дальнейшие исследования для оптимизации диагностики, терапии и повышения качества жизни таких пациентов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Erectile dysfunction (ED) and associated reproductive disorders (RD) are a significant medical and social problem, especially in patients with chronic kidney disease (CKD). In this category of patients, the frequency of erectile and reproductive disorders is significantly higher than in the general population, which negatively affects their quality of life and psychoemotional state.</p></sec><sec><title>Objective</title><p>Objective. The purpose of this review is to systematize current data on the prevalence, pathogenetic mechanisms and consequences of ED in patients with CKD and in kidney transplant recipients, as well as to analyze the effect of kidney transplantation (KT) on the restoration of sexual and reproductive function.</p></sec><sec><title>Material and methods</title><p>Material and methods. The review includes homeland and foreign studies mostly published in the recent 5 years, which have devoted to the assessment of erectile function in patients with CKD and after KT. Particular attention is paid to the use of standardized assessment methods, such as the International Index of Erectile Function (IIEF-5), as well as the analysis of data on hormonal background, psychoemotional and social aspects.</p></sec><sec><title>Conclusion</title><p>Conclusion. ED is a common complication of CKD caused by vascular, hormonal and psychoemotional factors. Effective treatment requires an interdisciplinary approach taking into account the somatic and psychological state of the patient. KT improves sexual function, but in some patients ED persists due to immunosuppression and concomitant diseases.
