Оценка эффективности и безопасности нового препарата инклисирана (непатентованное название, Novartis Overseas investments, AG (Швейцария)) у пациентов с хронической болезнью почек (ХБП) и трансплантированной почкой с нарушением липидного обмена.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Представлены первые результаты ретроспективного исследования применения инклисирана в серии клинических наблюдений у 3 пациентов с сочетанными заболеваниями: ишемической болезнью сердца (ИБС), нарушением липидного обмена и ХБП с трансплантированной почкой, имеющих очень высокий риск развития сердечно-сосудистых осложнений. Все пациенты получали комбинированную гиполипидемическую терапию (статины и эзетимиб), однако целевой уровень липопротеидов низкой плотности (ЛНП) достигнут не был.</p></sec><sec><title>Результаты</title><p>Результаты. Пациентам было проведено общее клиническое обследование. Оценивали биохимические показатели, экскреторную функцию почек и показатели липидного обмена (общий холестерин, ЛНП, липопротеиды высокой плотности, триглицериды). Оценку проводили до введения инклисирана, через 3 месяца после первого введения, через 6 месяцев после второго введения и перед третьим введением препарата. На этапах исследования было установлено стабильное снижение уровня общего холестерина на 18,4–19,1% и ЛНП на 51–59,6% от исходного уровня. Уровень липопротеидов высокой плотности и триглицеридов существенно не изменялся. Коррекцию иммуносупрессивной терапии не проводили. Неблагоприятных событий и осложнений в ходе исследования установлено не было. Целевой уровень ЛНП был достигнут.</p></sec><sec><title>Заключение</title><p>Заключение. Первые данные исследования по применению инклисирана у пациентов с ИБС, ХБП с трансплантированной почкой и нарушениями липидного обмена свидетельствуют о его эффективности и безопасности.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective. To evaluate the efficacy and safety of the new drug inclisiran (Novartis Overseas investments, AG (/witzerland)) in patients with chronic kidney disease (CKD) and transplanted kidney with impaired lipid metabolism.</p></sec><sec><title>Material and methods</title><p>Material and methods. The first results of a prospective case series study in three patients with combined diseases: cardiovascular diseases (CVD), lipid metabolism disorder and CKD with transplanted kidney, who have a very high risk of cardiovascular complications are presented. All patients received maximum combined lipid-lowering therapy (statins and ezetimibe) and immunosuppressive therapy. With this therapy, the target level of low-density lipoproteins was not achieved.</p></sec><sec><title>Results</title><p>Results. The patients underwent general clinical examination. Biochemical parameters, renal excretory function, immunosuppressive therapy, lipid metabolism parameters (total cholesterol, low-density lipoproteins, high-density lipoproteins, triglycerides) were evaluated. The evaluation was carried out before the administration of inclisiran, 3 months after the first administration, six months after the second administration and before the third administration of inclisiran. At the study stages, a stable decrease by 18.4-19.1% in total cholesterol and 51-59.6% in low-density lipoproteins from baseline was found. The level of high-density lipoproteins and triglycerides did not change significantly. Correction of immunosuppressive therapy was not performed. No adverse events and complications were detected during the study. The target level of low-density lipoproteins was achieved.</p></sec><sec><title>Conclusion</title><p>Conclusion.
