Острое расслоение аорты является актуальной и грозной проблемой современной кардиохирургии. В ранние сроки расслоения перед хирургом стоит дилемма выбора объема вмешательства, от минимальной реконструкции на восходящем отделе до радикального расширения объема вмешательства на дуге аорты. В настоящее время активно разрабатываются гибридные системы одномоментной реконструкции грудной аорты. Методика Frozen Elephant Trunk (FET) позволяет из классического стернотомного доступа заменить восходящий отдел, дугу аорты и эндопротезировать нисходящий отдел аорты дистальнее перешейка. Тем самым не увеличивается время операции, отсутствует этапность реконструкции участков аорты, восстанавливается адекватный кровоток в нисходящей аорте и ее ветвях, снижаются риски неблагоприятного исхода.</p></sec><sec><title>Цель</title><p>Цель. Провести анализ результатов хирургического лечения острого расслоения аорты А типа, выполненных по методике FET в условиях многопрофильной клиники НИИ скорой помощи им. Н.В. Склифосовского.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Исследуемую группу составили 18 пациентов с острым расслоением аорты типа А, оперированные в 2022–2024 годах. Всем пациентам было выполнено гибридное вмешательство по методике FET.</p></sec><sec><title>Результаты</title><p>Результаты. Синдром полиорганной недостаточности развился у 5 больных (27,8%). Четырем пациентам (22,2%) потребовалось проведение заместительной почечной терапии вследствие острой почечной недостаточности. У 38,9% исследуемых длительная искусственная вентиляция легких осложнилась пневмонией. Церебральные осложнения наблюдались у 6 пациентов (33,3%). Течение послеоперационного периода осложнилось развитием септического процесса в 16,7% наблюдений. В исследуемой группе 30-дневная летальность составила 22,2%.</p></sec><sec><title>Вывод</title><p>Вывод. В нашем исследовании применение гибридных протезов позволило получить относительно удовлетворительные результаты в раннем послеоперационном периоде после реконструкции грудной аорты при ее остром расслоении.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Acute aortic dissection is a current and urgent problem in modern cardiac surgery. In the early stages of the dissection, the surgeon is faced with the dilemma of choosing the volume of intervention from the ascending part reconstruction only or radically ascending and aortic arch replacement. Hybrid systems for one-stage reconstruction of the thoracic aorta are currently being actively developed. The Frozen Elephant Trunk (FET) technique allow us to replacement ascending and arch of the aortae combined with antegrade stent grafting into the descending aorta from the classical sternotomy access. This type of operation doesn’t increase the time of the intervention, there isn’t stage-by-stage reconstruction of the aorta, adequate blood flow in the descending aorta and aortic vessels is restored, and the risks of an adverse outcome are reduced.</p></sec><sec><title>Objective</title><p>Objective. To analyze the results of surgical treatment of acute aortic dissection type A, performed using the FET technique in a multidisciplinary surgical hospital – N.V. Sklifosovsky Research Institute for Emergency Medicine.</p></sec><sec><title>Material and methods</title><p>Material and methods. The research included 18 patients which were operated from 2022 to 2024 in acute stage of aortic dissection. All patients were operated using a hybrid technique FET.</p></sec><sec><title>Results</title><p>Results. Multisystem organ failure developed in 5 patients (27.8%). Four patients (22.2%) required renal replacement therapy due to acute renal failure. In 38.9% of the subjects, prolonged artificial ventilation was complicated by pneumonia. Cerebral complications were observed in 6 patients (33.3%). Sepsis accompanied the course of the disease in 16.7% of cases. The 30-day mortality was 22.2%, in the study group.</p></sec><sec><title>Conclusion</title><p>Conclusion.
