Для расширения пула донорских сердец в качестве одного из направлений предлагается экстракорпоральная перфузия трансплантатов. Немаловажным аспектом, требующим изучения в эксперименте, является определение параметров перфузии, таких как ее продолжительность, а также температура и давление перфузата, что приобретает важность при ее пролонгировании.</p></sec><sec><title>Цель</title><p>Цель. Представить результаты разработки перфузионного модуля для экстракорпоральной противоишемической защиты донорского сердца.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Разработан в эксперименте на половозрелых беспородных крысах-самцах перфузионный модуль для экстракорпоральной защиты донорского сердца. После 12-часовой гипотермической перфузии изолированных сердец при +8°С в перфузионном модуле оксигенированным раствором НТК исследовали их функцию.</p></sec><sec><title>Результаты</title><p>Результаты. В группе гипотермической перфузии у всех сердец в ходе реперфузии отмечался синусовый ритм с частотой сердечных сокращений (ЧСС) 268,5 (256,2;279,0) уд/мин; давление, развиваемое левым желудочком (ДРЛЖ) составило 65,5 (65,0;70,7) мм рт.ст., к концу 1-го часа реперфузии – 73,0 (70,5;75,0) мм рт.ст., через 90 минут – 82,0 (80,5;83,5) мм рт.ст., к концу 2-го часа реперфузии – 82,5 (80,5;84,0) мм рт.ст., (p&lt;0,001 по сравнению с контрольной группой), что свидетельствует о кардиопротективном потенциале гипотермической перфузии. У сердец контрольной группы отмечалась синусовая брадикардия с ЧСС 113,5 (90,0;124,7) уд/ мин. Сердца в контрольной группе демонстрировали развитие постишемической контрактуры со снижением ДРЛЖ до 27,5 (25,5;30,0) мм рт.ст. к окончанию периода 30-минутной стабилизации сердца, к концу 1-го часа реперфузии ДРЛЖ составляло 22,0 (20,5;23,7) мм рт.ст.; к 90-й минуте реперфузии сократительная функция сердец контрольной группы отсутствовала, что свидетельствует о развитии повреждения миокарда.</p></sec><sec><title>Выводы</title><p>Выводы. Полученные данные свидетельствуют, что перфузионный модуль обеспечивает поддержание жизнеспособности донорского сердца в течение 12 часов путем гипотермической перфузии через аорту оксигенированным раствором НТК при +8°С с объемной скоростью 0,3 мл/мин при давлении 10 см вод.ст. и уровне pO2 перфузата 600–700 мм рт.ст., что сопровождается восстановлением синусового ритма с ЧСС 325,0 (322,7;333,2) уд/мин и увеличением ДРЛЖ к концу 2-го часа реперфузии до 82,5 (80,5;84,0) мм рт.ст. (p&lt;0,001) в сравнении со статической консервацией.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Extracorporeal perfusion of transplants is proposed as one of the areas to expand the pool of donor hearts. Important aspects are the establishment of optimal perfusion parameters, which is of particular importance in prolonged perfusion.</p></sec><sec><title>Objective</title><p>Objective. To present the results of the development of a perfusion module for extracorporeal anti-ischemic protection of donor hearts.</p></sec><sec><title>Material and methods</title><p>Material and methods. A perfusion module for extracorporeal anti-ischemic protection of donor hearts was developed on mature outbred male rats. After 12-hour hypothermic extracorporeal perfusion of the heart in the perfusion module with +8°С oxegenated HTK solution, its function was studied.</p></sec><sec><title>Results</title><p>Results. In the hypothermic perfusion group on the module, sinus rhythm was observed with a heart rate of 268.5 (256.2;279.0) beats/min; the left ventricle-developed pressure (LVDP) was 65.5 (65.0;70.7) mm Hg, by the end of the 1st hour, the LVDP was 73.0 (70.5;75.0) mm Hg, after 90 minutes the LVDP was 82.0 (80.5;83.5) mm Hg, by the end of the 2nd hour, the LVDP was 82.5 (80.5;84.0) mm Hg, which indicated the cardioprotective potential of hypothermic perfusion. The hearts of the control group had sinus bradycardia with a heart rate of 113.5 (90.0;124.7) beats per minute. The LVDP was 27.5 (25.5;30.0) mm Hg by the end of the 30-minute cardiac stabilization period; by the end of the 1st hour, LVDP was 22.0 (20.5;23.7) mm Hg; by the 90th minute the contractile function of the hearts was absent, which indicated the development of myocardial damage.</p></sec><sec><title>Conclusion</title><p>Conclusion. Obtained data indicate that the module can maintain the viability of the donor heart for 12 hours by hypothermic perfusion with +8°С oxygenated HTK solution at flow rate of 0.3 ml/min and pressure of 10 cm H2O and pO2 600-700 mm Hg, which is accompanied by heart rate of 325.0 beats/min and LVDP of 82.5 mm Hg, compared to non-perfusion conservation.</p></sec></trans-abstract><kwd-group xml:lang="ru"><kwd>перфузионный модуль</kwd><kwd>экстракорпоральная перфузия</kwd><kwd>донорское сердце</kwd><kwd>гипотермия</kwd></kwd-group><kwd-group xml:lang="en"><kwd>perfusion module</kwd><kwd>extracorporeal perfusion</kwd><kwd>donor heart</kwd><kwd>hypothermia</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Chew HC, Macdonald PS, Dhital KK.
