Одним из поздних осложнений трансплантации печени является послеоперационная грыжа брюшной стенки, которая встречается у 5–46% реципиентов. Инцизионная грыжа снижает качество жизни, сопровождается различными осложнениями: спаечная болезнь, нарушение кишечного пассажа, ущемление. Помимо других факторов, влияющих на формирование рубца брюшной стенки, предикторами грыжеобразования считаются травматичные доступы.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Оценены результаты лечения 77 реципиентов после пересадки печени в период 2018–2025 гг. Родственная трансплантация была проведена 5 и трупная – 72 пациентам. J-образный доступ был использован у 75 (97,4%), типа “Mercedes” – у 2 (2,6%) реципиентов.</p></sec><sec><title>Результаты</title><p>Результаты. Инцизионная грыжа брюшной стенки осложнила поздний послеоперационный период в 38,9% наблюдений. Грыжевые дефекты были выявлены через 0,5–7 лет после трансплантации печени. В 50% наблюдений диагноз послеоперационной грыжи был установлен в течение 6–12 месяцев после операции. Слабым местом J-образного доступа является белая линия живота и угол послеоперационного рубца, где чаще появляются грыжевые дефекты.</p></sec><sec><title>Заключение</title><p>Заключение. Одной из причин грыжеобразования после трансплантации печени является разнонаправленный, довольно травматичный J-доступ, что предполагает оптимизацию методов укрепления «слабых» мест или применение других, преимущественно поперечных доступов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. One of the late complications of liver transplantation is postoperative hernia of the abdominal wall, which occurs in 5–46% of recipients. An incisional hernia reduces the quality of life, and is accompanied by various complications, such as adhesive disease, intestinal passage disorder, entrapment. In addition to other factors affecting the formation of an abdominal scar, traumatic surgical approaches are reviewed as predictors of herniation.</p></sec><sec><title>Material and methods</title><p>Material and methods. The results of treatment of 77 recipients after liver transplantation in the period 2018-2025 were evaluated. Related living donor transplantation was performed in 5 and cadaveric one in 72 patients. J-shaped approach was provided to 75 (97.4%) and “Mercedes” was used in 2 (2.59%) recipients.</p></sec><sec><title>Results</title><p>Results. An incisional hernia of the abdominal wall complicated the late postoperative period in 38.9% of cases. Hernial defect was diagnosed 0.5–7 years after transplantation, and in 50% of cases, the diagnosis of incisional hernia was made within 6 to 12 months after surgery. The locus minoris resistentiae of the J-shaped access is the white line and angle of the postoperative scar, where hernial defects appear more often.</p></sec><sec><title>Conclusion</title><p>Conclusion.
