Структурная дегенерация биологических протезов клапанов сердца – неизбежное осложнение в отдаленном периоде, требующее повторной операции. Повторная открытая операция сопряжена с высоким риском у коморбидных пациентов. Транскатетерная технология «клапан в клапан» (Valve-in-Valve, ViV) стала поэтому малоинвазивной альтернативой.</p></sec><sec><title>Цель</title><p>Цель. Продемонстрировать эффективность и безопасность двухэтапной стратегии ViV на трикуспидальном и митральном клапанах.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Представлено клиническое наблюдение пациентки 1947 года рождения с дисфункцией биопротезов митрального клапана (МК) и трикуспидального клапана (ТК) через 7 лет после первоначального открытого протезирования. Рассчитанный риск EuroSCORE II составил 21,8%. Была применена двухэтапная стратегия: 29.09.2025 года выполнена транскатетерная имплантация биопротеза MyVal, 30,5 мм в позицию ТК, а 10.10.2025 года – MyVal, 27,5 мм в позицию МК.</p></sec><sec><title>Результаты</title><p>Результаты. Послеоперационный период протекал без осложнений. Контрольная эхокардиография показала значительное улучшение гемодинамических параметров: средний градиент на ТК снизился с 6,0–7,2 до 3,1 мм рт.ст., регургитация уменьшилась со 2-й до 0–1-й степени. На МК средний градиент снизился с 9,0–10,5 мм рт.ст. до 6,7 мм рт.ст., регургитация была минимальной. Состояние пациентки при выписке удовлетворительное, функциональный класс регрессировал с III–IV до II.</p></sec><sec><title>Заключение</title><p>Заключение. Двухэтапная транскатетерная имплантация клапанов по технологии «клапан в клапан» является высокоэффективным и безопасным методом лечения дисфункции биопротезов у пациентов высокого хирургического риска, позволяющим избежать повторной стернотомии и сократить сроки реабилитации.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Structural degeneration of biological prosthetic heart valves is an inevitable complication in the long term, requiring repeated surgery. Repeated open surgery is associated with a high risk in comorbid patients. Valve-in-Valve (ViV) transcatheter technology has become a minimally invasive alternative.</p></sec><sec><title>Objective</title><p>Objective. To demonstrate the effectiveness and safety of a two-stage ViV strategy on tricuspid and mitral valves.</p></sec><sec><title>Material and methods</title><p>Material and methods. A clinical case of a patient born in 1947 with mitral valve (MC) and tricuspid valve (TC) bioprostheses 7 years after the initial open prosthetics is presented. The calculated risk of EuroSCORE II was 21.8%. A two-stage strategy was applied: on 09.29.2025, transcatheter implantation of the MyVal 30.5 mm bioprosthesis was performed in the tricuspid valve position, and on 10.10.2025, MyVal 27.5 mm was performed in the mitral valve position.</p></sec><sec><title>Results</title><p>Results. The postoperative period was uneventful. Control echocardiography showed a significant improvement in hemodynamic parameters: the average gradient on TC decreased from 6.0-7.2 to 3.1 mmHg, regurgitation decreased from 2 to 0-1 art. On MC, the average gradient decreased from 9.0-10.5 mmHg to 6.7 mmHg, regurgitation was minimal. The patient's condition was satisfactory upon discharge, FC regressed from III-IV to II.</p></sec><sec><title>Conclusion</title><p>Conclusion.
