Одним из приоритетных направлений в онкологии является разработка альтернативных органосохраняющих методов лечения при почечно-клеточном раке (ПКР) с внутрипаренхиматозной и центральной локализацией опухоли.</p></sec><sec><title>Цель исследования</title><p>Цель исследования: разработка метода экстракорпоральной резекции почки (ЭКРП) в условиях фармако-холодовой ишемии без пересечения мочеточника с последующей ортотопической аутотрансплантацией при ПКР.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование включены 37 пациентов с морфологически подтвержденным ПКР в стадии pT1a-Т3bN0M0-1G1-3 с интрапаренхиматозной и центральной локализацией опухоли.</p></sec><sec><title>Результаты</title><p>Результаты. Средняя длительность операции составила 413,97 ± 89,14 минуты. Средняя продолжительность холодовой ишемии почки – 151,41 ± 41,29 минуты, тепловой ишемии – 8,39 ± 4,75 минуты. Интраоперационные осложнения составили 8,1% (3), послеоперационные – 48,6% (18).</p></sec><sec><title>Заключение</title><p>Заключение. ЭКРП в условиях фармако-холодовой ишемии без пересечения мочеточника с последующей ортотопической аутотрансплантацией при ПКР позволяет выполнять резекции опухолей больших размеров, любой локализации в условиях длительной холодовой ишемии. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. One of the priorities in oncology includes the development of alternative organoprotection treatments for renal cell carcinoma (RCC) of intraparenchymal and central tumor location.</p><p>Study objective was to develop the method of extracorporeal kidney resection without ureter transection in conditions of pharmacological cold ischemia, followed by an orthotopic autotransplantation for RCC.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study included 37 patients who had a morphologically confirmed RCC of stage pT1a-T3bN0M0-1G1-3 with intraparenchymal and central tumour location.</p></sec><sec><title>Results</title><p>Results. The mean duration of surgery was 413.97 ± 89.14 minutes. The mean cold and warm ischemia time of kidney was 151.41 ± 41.29 minutes, and 8.39 ± 4.75 minutes, respectively. Intraoperative complications made 8.1% (3), postoperative complication rate was 48.6% (18).</p></sec><sec><title>Conclusion</title><p>Conclusion.
