В проспективное контролируемое рандомизированное исследование было включено 30 пациентов после ОТП. В 1-й группе (n=15) в течение 5 сут после операции вводили селен (Селеназа, Biosyn). Во 2-й группе (n=15) селен в комплексной терапии не применялся.</p></sec><sec><title>Результаты</title><p>Результаты. Уровень селена в плазме крови больных после ОТП значительно ниже нормального значения. В 1-й группе уровни аланинаминотрансферазы (АЛТ) и аспартатаминотрансферазы (АСТ) был достоверно ниже на 3-и сутки после ОТП, а к 7-м суткам достоверно снижались уровни мочевины и креатинина. При анализе тяжести состояния пациентов по шкале SOFA в послеоперационном периоде определялось достоверное различие в количестве баллов на 7-е сутки. Число пациентов, которым требовалось проведение вено-венозной гемодиафильтрации (ВВГДФ), было больше в группе, в которой селеназа не применялась.</p></sec><sec><title>Выводы</title><p>Выводы. Уровень селена значительно снижается после трансплантации печени. Селеназа обладает гепатопротекторным эффектом, уменьшает проявления системной воспалительной реакции, позволяет снизить частоту применения ВВГДФ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective: to study the effect of selenium on the course of a postoperative period in patients after orthotopic liver transplantation (OLT).</p></sec><sec><title>Subjects and methods</title><p>Subjects and methods. The prospective controlled randomized study enrolled 30 patients after OLT. Selenium (Selenase, Biosyn) was administered in Group 1 (n = 15) within 5 days operatively. The agent was not used in complex therapy in Group 2 (n = 15).</p></sec><sec><title>Results</title><p>Results. After OLT, the plasma levels of selenium were much more than the normal value. In Group 1, the levels of alanine aminotransferase (ALT) and aspartate aminotransferase (AST) were significantly lower on day 3 after OLT and those of urea and creatinine significantly decreased by day 7. A significant difference was found in scores on day 7, as shown by the analysis of the patients' condition by the SOFA scale. The patients who needed venovenous hemodiafiltration (VVHDF) were more than in those who did not use selenium.</p></sec><sec><title>Conclusion</title><p>Conclusion. Selenium levels considerably decrease after liver transplantation.
