Расширение критериев изъятия донорских органов способствует закономерному увеличению количества трансплантаций почек пожилым реципиентам, однако ввиду снижения требований к качеству донорского органа требует анализа результатов лечения.</p></sec><sec><title>Цель</title><p>Цель: проведение анализа и сравнение результатов трансплантаций почек пациентам пожилого возраста в зависимости от качества донорского органа.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Исследование основано на анализе результатов 61 трансплантации почек пожилым реципиентам, 51 из которой была выполнена от доноров с расширенными критериями (1-я группа) и 10 – от стандартных доноров (2-я группа). Сравнивали результаты восстановления функции, выживаемость трансплантатов/реципиентов, причины потерь почечных трансплантатов в раннем послеоперационном периоде на основании клинических, лабораторных, гистологических и инструментальных методов диагностики.</p></sec><sec><title>Результаты</title><p>Результаты. У пациентов 1-й группы по сравнению с больными 2-й группы статистически значимо чаще наблюдались отсроченная начальная функция (37,3% и 10% соответственно), отсутствие восстановления функции трансплантата почки (15,7% и 0%), а также более низкая ранняя послеоперационная выживаемость трансплантатов (72,5% и 100%). Восстановление функции почечного трансплантата отмечено у 58,8% пациентов 1-й группы и у всех больных 2-й группы. Наиболее частой причиной утраты почечного трансплантата и проведенных нефротрансплантатэктомий в раннем послеоперационном периоде являлось неудовлетворительное качество пересаженного органа по причине донорской патологии.</p></sec><sec><title>Вывод</title><p>Вывод. Доказаны статистически значимое ухудшение начальной функции трансплантата почки, увеличение частоты первично нефункционирующих трансплантатов, уменьшение выживаемости нефротрансплантатов в раннем послеоперационном периоде у реципиентов пожилого возраста при трансплантации почек от доноров с расширенными критериями.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction: The expansion of the criteria for donor organ retrieval contributes to an increase in the number of kidney transplantations to elderly recipients; but in view of reduced requirements to donor organ quality, a further analysis of transplantation outcomes is needed. The aim was to analyze and compare the outcomes of kidney transplantation to elderly patients depending on the donor organ quality.</p></sec><sec><title>Material and methods</title><p>Material and methods. The study was based on the analysis of the kidney transplantation outcomes in 61 elderly recipients, including 51 transplantations performed from expanded criteria donors (group 1), and other 10 from standard donors (group 2). Based on clinical, laboratory, histological, and instrumental diagnostic data, we compared the graft function recovery rates, graft/recipient survival rates, the causes of graft loss in the early posttransplant period.</p></sec><sec><title>Results</title><p>Results: Patients of group I had significantly higher delayed graft function rates (37.3% vs. 10%), graft non-function rates (15.7% vs. 0%), and lower early posttransplant survival rates (72.5% vs. 100%). Graft function recovery rate was 58.8% in group I, and 100% in the patients of group II. The most common cause of the graft loss and the renal graft removals performed in the early posttransplant period was the poor graft quality due to the donor's existing pathology.</p></sec><sec><title>Conclusion</title><p>Conclusion.
