Оценка скорости клубочковой фильтрации имеет существенную значимость, так как помимо объективного показателя функциональных возможностей почек она также является предиктором различных патологических состояний, в том числе сердечно-сосудистых осложнений.</p></sec><sec><title>Цель исследования</title><p>Цель исследования: анализ уровня креатинина и расчет скорости клубочковой фильтрации до и в отдаленные сроки после ортотопической трансплантации печени в клинической практике.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Проведен ретроспективный анализ данных пациентов (n = 89), оперированных в ГБУЗ«НИИ–ККБ № 1 им. проф. С.В. Очаповского» Краснодара. Исследовали уровень креатинина в крови и скорость клубочковой фильтрации пациентов до трансплантации печени, а также спустя 6, 12, 24 и 36 месяцев после операции. Статистический анализ результатов исследования проводили с использованием программы Statistica 10.</p></sec><sec><title>Результаты</title><p>Результаты. До трансплантации печени уровень креатинина в плазме крови составлял 82,9 ± 19,8 ммоль/л, через 6 месяцев зарегистрировано увеличение креатинина на 20,4% (р = 0,004), через 12, 24 и 36 месяцев на 24,8% (р = 0,00001), 24,4% (р = 0,0004) и 26,0% (р = 0,0005) соответственно статистически значимо во всех случаях. Снижение скорости клубочковой фильтрации произошло в сравнении с исходным на 14,2% (р = 0,0005), 18,8% (р= 0,00001), 20,2% (р = 0,00003) и 22,6% (р = 0,00006) через 6, 12, 24 и 36 месяцев соответственно статистически значимо во всех случаях. Статистически значимые различия функции почек между мужчинами и женщинами, а также у реципиентов младше и старше 50 лет отмечались только через год после трансплантации.</p></sec><sec><title>Выводы</title><p>Выводы. В отдаленном послеоперационном периоде наблюдаются повышение уровня креатинина и снижение скорости клубочковой фильтрации в сравнении с периодом до трансплантации печени. Через 6 месяцев, 2 и 3 года после трансплантации печени не выявлено статистически значимых различий в зависимости от возраста, а также между группами мужчин и женщин. Мониторирование скорости клубочковой фильтрации и профилактика прогрессирования почечной дисфункции у реципиентов печени необходимы независимо от возраста и пола.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Glomerular filtration rate is the main parameter characterizing the kidney functional condition. The evaluation of glomerular filtration rate is of significant importance, since in addition to an objective indicator of the kidney functional capacity, it is also a predictor of various pathological conditions, including cardiovascular complications.</p><p>The aim of the study was to assess creatinine levels and glomerular filtration rates before and after orthotopic liver transplantation in clinical practice.</p></sec><sec><title>Material and methods</title><p>Material and methods. A retrospective analysis of data obtained at different stages of the postoperative period in patients (n = 89) who underwent orthotopic liver transplantation in Research Institute – Regional Clinical Hospital № 1, Krasnodar, was performed. The creatinine level and glomerular filtration rate were studied before and at 6, 12, 24, and 36 months after liver transplantation, taking into account gender and age differences. Statistical analysis of the study results was made using Statistica 10 software.</p></sec><sec><title>Results</title><p>Results. The creatinine level was 82.9 ± 19.8 mmol/L before liver transplantation; creatinine increased by 20.4% (p = 0.004) at 6 months, by 24.8% (p = 0.00001), 24.4% (p = 0.0004), and 26.0% (p = 0.0005) at 12, 24 and 36 months after transplantation, respectively statistical significant in all cases. Compared to the baseline, the glomerular filtration rate decreased by 14.2% (p = 0.0005), 18.8% (p = 0.00001), 20.2% (p = 0.00003), and 22.6% (p = 0.00006) at 6, 12, 24, and 36 months, respectively statistical significant in all cases.
