Перемещение васкуляризированных костных трансплантатов является одним из самых надежных хирургических методов лечения несросшихся переломов и дефектов костей конечностей. Однако при выполнении таких оперативных вмешательств имеется ряд трудностей, одной из которых является сложность подгонки размеров реципиентного ложа и трансплантата, а также восстановления оси и длины реконструируемой кости. Перспективным методом подготовки к таким вмешательствам является виртуальное трехмерное планирование на основе данных компьютерной томографии и печать направителей и шаблонов. </p></sec><sec><title>Цель</title><p>Цель. Обобщение клинических наблюдений лечения несросшихся переломов и дефектов костей конечностей с помощью васкуляризированных костных аутотрансплантатов. </p></sec><sec><title>Материал и методы</title><p>Материал и методы. Оценены ход лечебного процесса и его результаты у 4 пациентов с несращениями костей конечностей и у 6 пациентов с дефектами, у которых применены васкуляризированные костные трансплантаты в сочетании с внутренним остеосинтезом в 7 наблюдениях и внешним остеосинтезом – в 3 наблюдениях. 4 пациентам в ходе подготовки к оперативным вмешательствам применено виртуальное трехмерное планирование и печать направителей и шаблонов. </p></sec><sec><title>Результаты</title><p>Результаты. В 1 наблюдении отмечен некроз васкуляризированного костного транспланатата, обусловленный венозным тромбозом. Сращения удалось добиться у всех пациентов, в 2 наблюдениях отмечена замедленная консолидация перелома. В 2 случаях выявлена гематома донорской области. При оценке продолжительности оперативного вмешательства выявлено ее снижение в среднем на 1 ч 5 мин в наблюдениях, где трехмерное виртуальное планирование применялось, по сравнению с наблюдениями, в которых данная методика не использовалась. При оценке анатомических результатов операции среди пациентов, которым не проводили трехмерное планирование, выявлено 2 случая неудовлетворительной репозиции. В 4 наблюдениях, в которых выполняли трехмерное планирование оперативного вмешательства, случаев неудовлетворительной репозиции не отмечено. </p></sec><sec><title>Выводы</title><p>Выводы. Применение васкуляризированных костных трансплантатов является эффективной методикой для достижения консолидации при дефектах и несращениях костей конечностей. Однако имеются трудности при планировании оперативных вмешательств, определении размеров донорского ложа и размеров трансплантатов. Наши предварительные результаты показывают, что виртуальное трехмерное предоперационное планирование и печать шаблонов позволяют повысить точность проведения вмешательств и сократить их длительность. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Vascular bone graft transposition is the one of most effective method of nonunion fracture and bone defect treatment. However, the use of this technique is associated with some difficulties. One is the adjustment of recipient bed size and the graft. The other is the difficulty to reconstruct the alignment and length of bone. A promising method of preparing for the vascular bone graft transposition is virtual three-dimensional planning based on computed tomography data and three-dimensional printing templates.</p><p>The aim was to summarize our experience in the treatment of bone nonunion and defects with vascular bone autografts using tree-dimensional virtual planning and printing.</p></sec><sec><title>Material and methods</title><p>Material and methods. We analyzed the treatment process and outcomes of 4 patients with limb bone nonunion and 6 patients with bone defects. In all cases, we used vascular bone grafts. Internal fixation of grafts was used in 7 cases, external fixation was used in 3 cases. At preparation stage in 4 cases, we used tree-dimensional virtual surgery planning and printing templates.</p></sec><sec><title>Results</title><p>Results. One case was diagnosed with bone graft necrosis caused by venous thrombosis. Consolidation was achieved in all patients; a late consolidation was observed in 2 cases. Hematoma in donor area was seen in 2 patients. When using three-dimensional virtual planning and tree-dimensional printing templates, the operation time was decreased by 1 hour 5 minutes. We identified two cases of poor reposition in the group without virtual planning. No poor reposition was observed in the cases where tree-dimensional planning was used.</p></sec><sec><title>Conclusion</title><p>Conclusion. Vascularized bone grafts provide an effective method to treat bone defects and nonunion. But the planning of graft and recipient site sizes is associated with certain difficulties.
