Саркопения является частым осложнением хронических прогрессирующих болезней печени. В этой связи множество современных научных работ посвящено изучению влияния саркопении на течение заболеваний печени. В то же время множество работ проводится и в отношении корреляционной взаимосвязи саркопении у пациентов с циррозом печени и по частоте ранних осложнений трансплантации печени. В этой связи мы провели собственное ретроспективное исследование корреляционной взаимосвязи саркопении у пациентов c хроническими заболеваниями печени.Цель. Определить корреляционную взаимосвязь степени саркопении с частотой развития осложнений цирроза печени у пациентов, находящихся в листе ожидания трансплантации печени.Материал и методы. В собственное ретроспективное обсервационное исследование были включены 87 пациентов, страдающих хроническими заболеваниями печени и проходивших лечение в ГУ «МНПЦ хирургии, трансплантологии и гематологии». Оценку саркопении проводили с использованием компьютерно-томографической методики оценки индексированных показателей оценки мышечной массы пациента. Математическую обработку проводили с использованием одобренного FDA ПО OsirixTM (Pixmeo SARL, Швейцария). Исследование проводили в рамках проекта «ClinicalTrials: NCT04281797».Результаты. Индекс массы тела имеет значимую корреляционную взаимосвязь с наличием саркопении (rpb=-0,48, 95%CI [-0,65;-0,27]). При этом группы пациентов с и без саркопении существенно различались (p&lt;0,0001), (g Хеджеса=-0,93, 95%CI [-1,37;-0,48]). В группах пациентов с различными этиологичесскими факторами хронических заболеваний печени саркопения встречалась чаще всего среди пациентов с аутоиммунными гепатитами, при этом неравномерное распределение по частоте возникновения саркопении в различных нозологических группах носило значимый характер (p=0,028; 2=14,1).Заключение. Индекс массы тела важен в диагностике прогрессивной потери мышечной массы у пациентов с хроническими прогрессирующими болезнями печени. Однако корреляционная взаимосвязь индексированных показателей скелетной мускулатуры и индекса массы тела существенно отличается между пациентами с саркопенией и без. Кроме того, в нашем исследовании была показана статистически значимая разница возникновения саркопении у пациентов, находящихся в листе ожидания в различных этиологических группах. Таким образом, дальнейшие широкие рандомизированные исследования в указанном направлении сегодня представляются крайне актуальными.</p></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Sarcopenia is a common complication of chronic liver disease. Today many studies are devoted to sarcopenia impact on the course of liver diseases. At the same time, many studies are being performed on the correlation of sarcopenia in patients with cirrhosis of the liver and the incidence of early complications after liver transplantation. In this regard, we conducted our own retrospective study of the correlation relationship of sarcopenia in patients with chronic liver diseases.</p></sec><sec><title>Aim</title><p>Aim. To determine the correlation between the severity of sarcopenia and the incidence of complications of liver cirrhosis in patients on the waiting list for liver transplantation.</p></sec><sec><title>Material and methods</title><p>Material and methods. In our own retrospective observational study, 87 patients suffering from chronic liver diseases and treated at the Minsk Scientific and Practical Center of Surgery, Transplantation and Hematology were included. The assessment of sarcopenia was carried out using the CT technique for assessing indexed parameters of the patient's muscle mass. Mathematical processing was performed using the FDA-approved OsirixTM software. The study was conducted within the framework of the project "ClinicalTrials: NCT04281797"</p></sec><sec><title>Results</title><p>Results. Body mass index was significantly correlated with the presence of sarcopenia (rpb=-0.48, 95% CI [-0.65;-0.27]). At the same time, the groups of patients with and without sarcopenia differed significantly (p&lt;0.0001), (Hedges g=-0.93, 95% CI [-1.37;-0.48]). In groups of patients with various etiological factors of chronic liver diseases, sarcopenia occurred</p><p>most often among patients with autoimmune hepatitis, while the uneven distribution of the incidence of sarcopenia in different nosological groups was significant (p=0.028; 2=14.1).</p></sec><sec><title>Conclusion</title><p>Conclusion. Body mass index plays an important prognostic role in muscle mass loss among patients with clinically advanced liver disease. However, skeletal muscle index and Body Mass Index differ significantly between patients with and without sarcopenia. In addition, our study showed a significant difference in the occurrence of sarcopenia in patients on the waiting list of different etiological groups.
