Отравления парацетамолом часто встречаются во всем мире, в том числе и в России. В 20–25% случаев при этом наблюдается прием массивной дозы препарата – более 30–40 г одномоментно.Цель. Продемонстрировать эффективность применения повышенных доз ацетилцистеина при массивном приеме парацетамола.Результаты. Пациентка Г., 22 лет, приняла с целью суицида 70 таблеток (35 г) парацетамола за 3 часа до поступления в стационар. Уровень парацетамола в крови через 4 часа после его приема составил 694,94 мкг/мл. При поступлении в стационар было начато введение ацетилцистеина по 12-часовой схеме. Впоследствии продолжено введение ацетилцистеина по 20-часовой схеме с повышенной дозировкой на 2-м этапе. Лабораторные показатели, в том числе уровней в крови аспартатаминотрансферазы и аланинаминотрансферазы, во время нахождения в стационаре оставались в пределах референтных значений.Заключение. Представленное нами наблюдение показывает эффективность и целесообразность применения повышенных доз ацетилцистеина при массивном приеме парацетамола, что способствует предупреждению тяжелых осложнений, благоприятному течению и исходу заболевания.</p></abstract><trans-abstract xml:lang="en"><p>Background. Paracetamol poisoning is common all over the world, including in Russia. In 20–25% of cases, a massive dose of the drug is observed: more than 30–40 g of paracetamol at a time.The aim of the study was to demonstrate the efficacy of using an increased doses of acetylcysteine in the treatment of a massive paracetamol admission.Results. Patient G., 22 years old, took 70 tablets (35 g) of paracetamol for suicide 3 hours before admission to the hospital. The blood level of paracetamol 4 hours after taking it was 694.94 µg/mL. Upon admission to the hospital, acetylcysteine administering was started according to a 12-hour scheme. Subsequently, the administration of acetylcysteine was continued according to a 20-hour regimen with an increased dosage at the 2nd stage. Laboratory parameters, including aspartate aminotransferase and alanine aminotransferase, remained within the reference values during hospital stay. Conclusion.
