Инфекционные осложнения способствуют значимому снижению показателей выживаемости не только трансплантатов, но и реципиентов. В статье описан клинический случай пересадки первично инфицированного трансплантата почки. Материал и методы. Пациенту 33 лет с терминальной хронической почечной недостаточностью в исходе диабетической нефропатии выполнили аллогенную гетеротопическую трансплантацию почки. Посмертным донором был 46-летний мужчина с констатированной смертью головного мозга в исходе острого нарушения мозгового кровообращения. Результаты. Ранний послеоперационный период осложнился первичным инфицированием почечного трансплантата. Несмотря на проводимые мероприятия, направленные на сохранение трансплантированной почки, мы были вынуждены удалить ее с целью предотвращения развития дальнейших осложнений у реципиента. Заключение. Трансплантация почки способствует повышению качества жизни пациентов с терминальной стадией хронической почечной недостаточности. При неконтролируемом течении инфекционного процесса после пересадки первично инфицированного трансплантата необходимо своевременно выполнять нефротрансплантатэктомию с целью сохранения жизни реципиента, поскольку сам трансплантат является источником инфекции.</p></abstract><trans-abstract xml:lang="en"><p>Introduction. Infectious complications contribute to a significant decrease in graft and recipient survival rates. The article describes a case report of transplantation of the primary infected kidney transplant. Material and methods. A 33-year-old patient with type 1 diabetes mellitus and end-stage renal disease underwent kidney transplantation. The deceased donor was a 46-year-old man with a confirmed brain death as a result of acute cerebrovascular accident. Results. The early postoperative period was complicated by the development of primary infection of kidney transplant. Despite the ongoing treatment aimed at preserving the transplant, we had to remove it in order to prevent the development of further complications. Conclusion. Kidney transplantation improves the quality of life of patients with end-stage renal disease.
