Послеоперационная аналгезия у реципиентов трансплантата почки представляет собой сложную задачу из-за нефротоксичности нестероидных противовоспалительных препаратов и снижения клиренса метаболитов опиоидов при дисфункции почки. Опиоидсберегающая мультимодальная послеоперационная аналгезия с использованием методов регионарной аналгезии может обеспечить лучший контроль боли и раннюю активизацию после трансплантации почки.</p></sec><sec><title>Цель</title><p>Цель. Оценить клинические результаты применения регионарных методов обезболивания у реципиентов почечного трансплантата.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Проведено одноцентровое исследование в Республиканском научном центре экстренной медицинской помощи в период с 2020 по 2022 г. В исследование включены 97 пациентов, перенесших гетеротопическую трансплантацию почки от живого родственного донора, которые были разделены на три группы. В 1-й группе (31 пациент) применяли общую комбинированную анестезию. Послеоперационное обезболивание осуществляли наркотическими анальгетиками в сочетании с метамизолом в дозе 1000 мг. Во 2-й группе (33 пациента) применяли сочетание общей анестезии и открытой блокады пространства поперечной мышцы живота (transversus abdominis plane block). В 3-й группе (33 пациента) проводили сочетание общей анестезии и блокады пространства мышцы, выпрямляющей спину (erector spine plane block). Наркотические анальгетики применяли в качестве «спасительной аналгезии» при необходимости. Первичными конечными точками исследования были интенсивность боли по визуально-аналоговой шкале и потребление наркотического анальгетика в морфиновом эквиваленте в первые сутки после операции. Вторичными конечными точками явились время восстановления перистальтики кишечника, наличие тошноты и рвоты, длительность нахождения в реанимационном отделении и стационаре.</p></sec><sec><title>Результаты</title><p>Результаты. Интенсивность боли через 6 часов после операции у пациентов 1-й группы была на 13,5% и 24,6% выше, чем у пациентов 2-й и 3-й групп соответственно. Во 2-й группе интенсивность боли была на 12,8% выше по сравнению с 3-й группой (р=0,0017). Через 12 часов после операции в 1-й группе интенсивность боли была на 42% больше по сравнению с 2-й и 3-й группой (p&lt;0,0001). Через 18 часов показатель боли в 3-й группе был ниже на 48,5% и 35,7% по сравнению с 1-й и 2-й группами соответственно (p&lt;0,0001 и p=0,0016). Через 24 часа после операции в 1-й группе ощущение боли было на 18,6% и 65,3% выше по сравнению с 2-й и 3-й группой (p&lt;0,0001). Средняя доза наркотического анальгетика, эквивалентного морфину, в 1-й группе, составила 22,6±8,6 мг, что на 18,5% выше, чем у пациентов 2-й группы. В 3-й группе данный показатель составил 12,0±4,3 мг и был на 47% ниже по сравнению с 1-й группой (p&lt;0,0001) и на 34,7% ниже по сравнению со 2-й группой (p&lt;0,0001) (все сопоставления статистически значимы). Адекватность обезболивания на фоне низкого потребления наркотических анальгетиков способствовала отсутствию послеоперационной тошноты и рвоты в 75% случаев, раннему восстановлению перистальтики кишечника на 63% по сравнению с группой пациентов, где применяли наркотические анальгетики для послеоперационного обезболивания.</p></sec><sec><title>Выводы</title><p>Выводы. Сочетанное применение общей анестезии и блокады пространства мышцы, выпрямляющей спину, может быть рекомендовано у пациентов–реципиентов почечного трансплантата в периоперационном периоде при трансплантации почки.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Postoperative analgesia in kidney transplant recipients is challenging due to potential nephrotoxicity of nonsteroidal anti-inflammatory drugs and the reduced clearance of opioid metabolites during transient renal impairment. Opioid-sparing multimodal postoperative analgesia using regional analgesia methods could provide better pain control and early activation after kidney transplantation.</p></sec><sec><title>Aim</title><p>Aim. To evaluate the clinical results of treatment using regional pain management methods in kidney transplant recipients.</p></sec><sec><title>Material and methods</title><p>Material and methods. A single-center study was conducted at Republican Research Center of Emergency Medicine from 2020 to 2022. The study included 97 patients who underwent heterotopic kidney transplantation from a living related donor. Patients were divided into 3 groups. In group 1 (31 patients), general anesthesia was used. For postoperative analgesia opioid analgesics in combination with metamizole 1000 mg were used. In group 2 (33 recipient patients), a combination of general anesthesia and open transversus abdominis plane block was used. In group 3 (33 recipient patients), a combination of general anesthesia and erector spine plane block was performed. Opioid analgesics were used as a "rescue analgesia" when necessary. The primary study end points were the pain intensity assessed by a visual analogue scale and opioid consumption on the first day after surgery. Secondary endpoints were the time of intestinal motility recovery, the presence of nausea and vomiting, the Intensive Care Unit length of stay and the hospital length of stay.</p></sec><sec><title>Results</title><p>Results. Pain intensity 6 hours after surgery in patients of group 1 was 13.5% and 24.6% higher than in patients of group 2 and 3, respectively. In group 2, pain intensity was 12.8% higher compared to group 3 (p=0.0017). At 12 hours after surgery, the pain intensity was 42% higher in group 1 compared to group 2 and group 3 (p&lt;0.0001). After 18 hours, the pain score in group 3 was 48.5% and 35.7% lower compared to groups 1 and 2, respectively (p&lt;0.0001 and p=0.0016). 24 hours after surgery, the sensation of pain was 18.6% and 65.3% higher in group 1 compared to groups 2 and 3 (p&lt;0.0001). The mean dose of narcotic analgesic equivalent to morphine in group 1 was 22.6±8.6 mg, which was 18.5% higher than in group 2 patients. In group 3, it was 12.0±4.3 mg and was 47% lower compared to group 1 (p&lt;0.0001) and 34.7% lower compared to group 2 (p&lt;0.0001) (all comparisons are statistically significant). The adequacy of analgesia and less opioid consumption contributed to the absence of postoperative nausea and vomiting in 75% of cases, early restoration of intestinal motility in 63% compared with the group of patients where opioid analgesics were used for postoperative pain relief.</p></sec><sec><title>Conclusion</title><p>Conclusion.
