Отек легких является частым осложнением у пациентов в критическом состоянии. Либеральная тактика инфузионной терапии критического состояния и патологическое накопление внесосудистой легочной жидкости увеличивают риски смертности. Своевременная и неинвазивная диагностика отека легких является одной из главных целей при интенсивной терапии пациента в отделении реанимации. Для диагностики отека легких мы предпочитаем использовать ультразвуковое исследование легких с регистрацией В-линий. Однако в нашей стране из-за отсутствия достаточного количества исследований и необходимой регламентирующей базы данный метод не валидирован.</p></sec><sec><title>Цель</title><p>Цель. Оценить возможность применения ультразвуковой диагностики отека легких у пациентов в критическом состоянии.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Проведено ретроспективное исследование 27 пациентов, среди которых было 15 мужчин и 12 женщин в возрасте от 43 до 67 лет, средний возраст 45,05±17,2 года. Все пациенты находились в критическом состоянии, обусловленном острой печеночной, либо острой на фоне хронической печеночной недостаточностью, либо ранней дисфункцией трансплантата печени, либо пострезекционной печеночной недостаточностью. У части пациентов имел место синдром системного воспалительного ответа с развитием полиорганной недостаточности и клиники перераспределительного шока. Всем пациентам выполняли ультразвуковое исследование легких, а также проводили измерение показателей гемодинамики методом транспульмональной термодилюции. Данные, полученные двумя методами, сравнивали между собой.</p></sec><sec><title>Результаты</title><p>Результаты. Определена статистически значимая взаимосвязь (p&lt;0,05) между показателем индекса внесосудистой легочной жидкости и наличием отека легких. Также мы выявили значимую взаимосвязь (p&lt;0,05) между количеством В-линий и наличием отека легких. Проведен корреляционный анализ взаимосвязи показателя «внесосудистой легочной жидкости» и показателя "B-линия", по результатам которого была установлена прямая связь высокой степени. Площадь под ROC-кривой составила 0,9±0,06 с 95% ДИ [0,77–1,00]. Полученная модель была статистически значимой (p=0,004).</p></sec><sec><title>Выводы</title><p>Выводы. Данные ультразвукового исследования при диагностике отека легких имеют значимую степень корреляции с уровнем внесосудистой легочной жидкости. Ультразвуковое исследование легких является точным, неинвазивным, безопасным методом оценки внелегочной жидкости и может быть использовано для быстрой, прикроватной диагностики отека легких.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Pulmonary edema is a common complication in critically ill patients. The liberal tactics of fluid therapy and pathological accumulation of extravascular lung water increase the risks of mortality in Intensive Care Unit patients. Timely and non-invasive diagnosis of pulmonary edema is a primary goal in the intensive care of patients in the Critical Care Unit. We prefer to use lung ultrasound with the registration of B-lines to diagnose lung edema. However, in our country, this method is not validated due to the lack of a sufficient number of clinical studies and necessary regulatory framework.</p></sec><sec><title>Objective</title><p>Objective. To assess the potential of diagnostic ultrasonography for pulmonary edema in critically ill patients.</p></sec><sec><title>Material and methods</title><p>Material and methods. A retrospective study was conducted on 27 patients, including 15 males and 12 females aged from 43 to 67 years old (mean age 45.05±17.2 years). All patients were in critical condition due either to acute liver failure, or acute-on-chronic liver failure, or early post-transplant liver graft failure, or posthepatectomy liver failure. Some patients had a systemic inflammatory response syndrome with the development of multiple organ failure and clinical signs of redistribution shock. All patients underwent ultrasound examination of the lungs, and had hemodynamic parameters measured using the transpulmonary thermodilution technique. The data obtained by the two diagnostic modalities were compared.</p></sec><sec><title>Results</title><p>Results. A significant correlation (p&lt;0.05) was found between the extravascular lung water index and the presence of lung edema. We identified a significant correlation (p&lt;0.05) between the number of B-lines and the presence of pulmonary edema. In assessing the relationship between the "B-line" parameter and the "EVLWI" parameter, a strong positive correlation was identified. The area under the ROC curve (AUC) was 0.9±0.06 with a 95% CI [0.77–1.00].</p></sec><sec><title>Conclusions</title><p>Conclusions. Ultrasound data in diagnosing pulmonary edema have a significant correlation with the level of extravascular pulmonary water. Lung ultrasound is an accurate, non-invasive method for assessing extravascular lung water.
