Золотым стандартом лечения сахарного диабета, осложненного хронической болезнью почек 5-й стадии в исходе диабетической нефропатии, является сочетанная трансплантация почки и поджелудочной железы. Одной из главных проблем клинической трансплантации поджелудочной железы является вопрос отведения панкреатического сока. Для того чтобы сохранить преимущества забрюшинного расположения трансплантата и избежать главного недостатка дуоденального дренирования – фатальных осложнений в случае необходимости удаления трансплантата – нами был предложен модифицированный способ забрюшинной трансплантации поджелудочной железы с отведением панкреатического сока в отключенную по Ру петлю тонкой кишки. Он позволяет снизить число тяжелых хирургических осложнений и увеличить выживаемость реципиентов. При использовании этого способа ранее не было возможности оценить состояние слизистой оболочки донорской двенадцатиперстной кишки и межкишечных анастомозов.</p></sec><sec><title>Цель</title><p>Цель. Продемонстрировать возможности эндоскопической оценки межкишечных анастомозов при выполнении забрюшинной трансплантации поджелудочной железы с тонкокишечным отведением панкреатического сока.</p></sec><sec><title>Результаты</title><p>Результаты. Представлен первый опыт эндоскопической оценки состояния межкишечного соустья и слизистой двенадцатиперстной кишки донора после забрюшинной трансплантации поджелудочной железы с формированием дуоденоеюнального анастомоза при высоком отключении по Ру петли тонкой кишки.</p></sec><sec><title>Заключение</title><p>Заключение. Представленное наблюдение демонстрирует возможность эффективного проведения диагностических эндоскопических вмешательств при выполнении забрюшинной трансплантации поджелудочной железы с тонкокишечным отведением экзокринного секрета.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Simultaneous pancreas and kidney transplantation is a gold standard in the treatment of diabetes mellitus complicated by stage 5 chronic kidney disease as a result of diabetic nephropathy. One of the main problems of clinical pancreas transplantation is the pancreas graft exocrine drainage. In order to preserve the advantages of the retroperitoneal graft location and avoid the main disadvantage of duodenal drainage, namely, fatal complications potentially arising in case of necessary graft removal, we have proposed a modified method of retroperitoneal pancreatic transplantation with exocrine drainage via a modified Roux-en-Y duodenojejunostomy. It reduces the number of severe surgical complications and increases the recipient survival rate. When this method was used in previous years, it was not possible to assess the condition of donor duodenum mucosa and interintestinal anastomoses.</p></sec><sec><title>Objective</title><p>Objective. To demonstrate the possibility of endoscopic assessment of interintestinal anastomoses when performing retroperitoneal pancreas transplantation with small intestine drainage of exocrine secretions.</p></sec><sec><title>Results</title><p>Results. The article presents the initial experience of endoscopic assessment of the interintestinal anastomoses and the mucous membrane of the donor duodenum after retroperitoneal pancreas transplantation with exocrine drainage via a Roux-en-Y duodenojejunostomy.</p></sec><sec><title>Conclusion</title><p>Conclusion.
