В настоящее время большинство трансплантаций производится от доноров со смертью мозга (СМ). Со смертью в большинстве случаев связана выраженная физиологическая нестабильность, которая может привести или к ухудшению функции донорского органа до эксцизии, или к потере донора. При активном или даже агрессивном ведении донора возможны управление и коррекция патофизиологических процессов в донорских органах, тем самым увеличивается число и улучшается функциональное состояние донорских органов.</p><p>Агрессивная стратегия ведения потенциальных доноров со СМ требует изменения не только в подходах к интенсивной терапии, но и философии в оценке проводимой работы. Несмотря на разработку и внедрение различных протоколов ведения доноров со СМ, оптимального сочетания целей, мониторинга, специфической терапии до настоящего момента пока не достигнуто.</p></abstract><trans-abstract xml:lang="en"><p>The lack of donor organs is the main factor limiting organ transplantation. Currently, the majority of transplants are taken from brain-dead donors. In most cases the brain death is associateed with a severe physiologic instability that may deteriorate the donor organ function prior to the excision or lead to the loss of the donor. An active or even aggressive management of a donor allows the control and correction of pathophysiological processes in donor organs, thus increasing the number and improving the functional condition of donor organs.</p><p>An aggressive management strategy for a potential donor with brain death requires to change both the approach to intensive care, and the philosophic aspect in the evaluation of this work.
