Трансплантация печени является одним из приоритетных вариантов лечения гепатоцеллюлярной карциномы при наличии цирроза печени, однако точное прогнозирование исходов после операции остается сложной задачей. Существующие модели прогнозирования часто не учитывают возраст пациентов и тип донора. Улучшенные модели, включающие эти параметры, могут повысить точность прогнозов и эффективность лечения, что крайне важно в условиях такого динамично развивающегося направления, как трансплантология.</p></sec><sec><title>Цель</title><p>Цель. Разработка прогностической модели исходов трансплантации печени у пациентов с гепатоцеллюлярной карциномой и циррозом печени.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В ретроспективное исследование вошли 69 пациентов с гепатоцеллюлярной карциномой на фоне цирроза печени, которым выполнена трансплантация печени в период с мая 2010 по декабрь 2022 года. Из них 42 пациента (61%) получили орган от живых, а 27 (39%) – от посмертных доноров. Исследование включало анализ уровня альфа-фетопротеина в крови, а также оценку группы радиологических (максимальный размер опухолевого узла, их количество) и гистологических параметров (максимальный размер опухолевого узла, их количество, наличие сосудистой инвазии). Для прогнозирования безрецидивной выживаемости использовалась модель регрессии Кокса. Результаты прогноза 5-летней безрецидивной выживаемости, возраст реципиента, тип донора, были повторно использованы в модели Кокса для прогнозирования общей выживаемости.</p></sec><sec><title>Результаты</title><p>Результаты. Разработаны четыре модели для прогнозирования безрецидивной и общей выживаемости на основе гистологических и радиологических данных, продемонстрировавшие высокую прогностическую ценность моделей с С-индексом на обучающих/тестовых данных 0,76/1; 0,73/1; 0,78/0,8; 0,6/0,8 соответственно. Все модели показали точность прогнозирования безрецидивной выживаемости, сравнимую с Миланскими критериями. Модели представлены в виде калькулятора и доступны на веб-ресурсе <ext-link xlink:href="https://nadit.ru/calculate_HCC" ext-link-type="uri">https://nadit.ru/calculate_HCC</ext-link>.</p></sec><sec><title>Выводы</title><p>Выводы. Разработанные прогностические модели являются важными инструментами для персонифицированного прогнозирования исходов после трансплантации печени по поводу гепатоцеллюлярной карциномы. Для повышения точности этих моделей необходимо дальнейшее объединение и валидация данных из различных медицинских центров, открытое научное сотрудничество.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Liver transplantation remains a priority treatment option for hepatocellular carcinoma in the presence of liver cirrhosis; yet precise outcome prediction post-operation continues to be a complex challenge. Existing prognostic model often overlook patient age and donor type. Enhanced models that incorporate these parameters can improve prediction accuracy and treatment efficacy, which is critically important in the dynamically evolving field of transplantation.</p></sec><sec><title>Objective</title><p>Objective. The aim of this study is to develop a prognostic model for liver transplantation outcomes in patients with hepatocellular carcinoma and liver cirrhosis.</p></sec><sec><title>Material and methods</title><p>Material and methods. This retrospective study included 69 patients with hepatocellular carcinoma on the background of liver cirrhosis who underwent liver transplantation between May 2010 and December 2022. Of these, 42 patients (61%) received organs from living donors, and 27 (39%) from deceased donors. The study involved analysis of alpha-fetoprotein levels in blood, as well as assessment of radiological (maximum tumor nodule size, number of nodules) and histological parameters (maximum tumor nodule size, number of nodules, presence of vascular invasion). Cox regression model was used to predict recurrence-free survival, and the results for five-year recurrence-free survival, recipient age, and donor type were reused in the Cox model to predict overall survival.</p></sec><sec><title>Results</title><p>Results. Four models for predicting recurrence-free survival and overall survival based on histological and radiological data were developed, demonstrating high prognostic value with C-indexes on training/test data of 0.76/1; 0.73/1; 0.78/0.8; 0.6/0.8 respectively. All models showed recurrence-free survival prediction accuracy comparable to the Milan criteria. The model outcomes are available as a calculator on the website <ext-link xlink:href="https://nadit.ru/calculate_HCC" ext-link-type="uri">https://nadit.ru/calculate_HCC</ext-link>.</p></sec><sec><title>Conclusion</title><p>Conclusion. The developed prognostic models are vital tools for personalized outcome prediction after liver transplantation for hepatocellular carcinoma.
