Легочная гипертензия широко распространена среди пациентов с терминальной стадией хронической болезни почек, оказывая значимое негативное влияние на течение посттрансплантационного периода, в том числе – при трансплантации почки от живого родственного донора.</p></sec><sec><title>Цель</title><p>Цель. Изучить распространенность легочной гипертензии среди больных терминальной стадией хронической болезни почек, определить ее значение в развитии ранней дисфункции почечного трансплантата.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Исследование основано на анализе результатов лечения 650 пациентов, которым была проведена трансплантация почки от живого родственного донора. В зависимости от функции трансплантата выделено две группы пациентов: I группа с ранней дисфункцией почечного трансплантата (n=82); II группа (n=79) – пациенты с первично функционирующим трансплантатом, подобранные по демографическим и клинико-лабораторным данным без статистически значимой разницы по сравнению с пациентами I группы (р&gt;0,5). Это позволило уравнять шансы в достижении конечной точки исследования (ранняя дисфункция трансплантата). Всем больным выполнялась трансторакальная эхокардиография, с расчетом среднего давления в легочной артерии. Проведен расчет относительного риска развития ранней дисфункции почечного трансплантата в зависимости от наличия легочной гипертензии различной степени.</p></sec><sec><title>Результаты</title><p>Результаты. Среди пациентов I группы легочная гипертензия была выявлена в 97,56% случаев (среднее давление в легочной артерии 48,26±18,63 мм рт.ст.), по сравнению с 86,08% случаев у пациентов с первично функционирующим трансплантатом (среднее давление в легочной артерии 31,92±16,11 мм рт.ст.) (р&lt;0,001). Наличие легкой степени легочной гипертензии повышало относительный риск развития ранней дисфункции трансплантата в 2,58 раза (95% ДИ [0,698–9,547]; р=0,174), умеренной степени – в 3,18 раза (95% ДИ [0,860–11,764]; р=0,064), тяжелой степени – в 5,91 раза (95% ДИ [1,644–21,24]; р&lt;0,001) по сравнению с данными пациентов без легочной гипертензии.</p></sec><sec><title>Выводы</title><p>Выводы. При выполнении трансплантации почки от живого родственного донора наличие у реципиента тяжелой степени легочной гипертензии ассоциировано с повышением риска развития ранней дисфункции трансплантата, что позволяет рассматривать легочную гипертензию как один из модифицируемых факторов риска развития данного осложнения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Pulmonary hypertension is a common complication among patients with end-stage renal disease and has a significant impact on the outcome of kidney transplantation, including during kidney transplantation from a living related donor.</p></sec><sec><title>Objective</title><p>Objective. To study the prevalence of pulmonary hypertension among patients with end-stage renal disease and to determine its impact on the development of early renal graft dysfunction.</p></sec><sec><title>Material and methods</title><p>Material and methods. The study was based on the analysis of treatment results in 650 patients who underwent kidney transplantation from a living related donor. Depending on the graft function, 2 groups of patients were identified: group I consisting of patients with early renal graft dysfunction (n=82); group II (n=79) that included the patients with a primary graft function who were selected by demographic and clinical laboratory data statistically comparable to patients of group I (p&gt;0.5). This allowed us to equalize the chances of achieving the study endpoint (early graft dysfunction). Transthoracic echocardiography was performed in all patients, with the calculation of the mean pulmonary artery pressure. The relative risk of early renal graft dysfunction was calculated depending on the presence of pulmonary hypertension and its severity.</p></sec><sec><title>Results</title><p>Results. Among group I patients, pulmonary hypertension was detected in 97.56% of cases (mean pulmonary artery pressure 48.26±18.63 mmHg), versus 86.08% in patients with a primary graft function (mean pulmonary artery pressure 31.92±16.11 mmHg) (p&lt;0.001). The presence of mild pulmonary hypertension increased the relative risk of early graft dysfunction by 2.58 times (95% CI [0.698-9.547]; p=0.174), moderate by 3.18 times (95% CI [0.860-11.764]; p=0.064), severe by 5.91 times (95% CI [1.644-21.241]; p&lt;0.001) compared with patients without pulmonary hypertension.</p></sec><sec><title>Conclusions</title><p>Conclusions. When performing kidney transplantation from a living donor, the presence of severe pulmonary hypertension in the recipient is associated with an increased risk of early graft dysfunction.
