Безопасное изъятие правой доли печени и восстановление артериального кровоснабжения печеночного трансплантата возможно только при полноценном понимании анатомии печеночной артерии у донора.</p></sec><sec><title>Цель</title><p>Цель. Описать новые и дополнить имеющиеся сведения об анатомических особенностях артериального кровоснабжения правой доли печени у донора в зависимости от расположения устья артерий, их числа, места отхождения артерии к 4-му сегменту печени, синтопии с окружающими анатомическими структурами. Материал и методы. В ФГБУ ГНЦ ФМБЦ им. А.И. Бурназяна с 2009 по 2021 год было проведено 306 трансплантаций печени от живого родственного донора. Проанализирована сосудистая анатомия 518 потенциальных доноров. Произведена оценка частоты встречаемости различных вариантов артериального кровоснабжения правой доли печени у потенциальных доноров.</p></sec><sec><title>Результаты</title><p>Результаты. Были выделены 11 типов артериального кровоснабжения правой доли печени и 7 подтипов анатомии артерии 4-го сегмента печени. Донорство правой доли печени и реконструкция артериального кровотока возможна при всех типах и подтипах кроме типа K и подтипа 7. Частота наблюдения типов и подтипов, при которых была выполнена реконструкция, составила: тип A, подтипы 1–5 (58,4%, 26,1%, 5,5%, 1,9%, 0,3%); тип B, подтип 1, 4, 5 (по 0,3% каждый); тип С, подтипы 1, 2 (2,9%, 1,3%); тип D, подтипы 1, 3 (по 0,3% каждый); тип Е, подтип 1 (0,6%), тип F–J, подтип 1 (по 0,3% каждый). Частота артериальных послеоперационных осложнений (11 наблюдений) составила 3,5% от общего числа операций (n=306) и 5,9% от общего числа пациентов с осложнениями (n=184). Летальность, связанная с патологией артерии трансплантата, составила 1,9% (6 наблюдений).</p></sec><sec><title>Выводы</title><p>Выводы. Были дополнены и детализированы существующие классификации артериального кровоснабжения правой доли печени в аспекте ее трансплантации. Выделены подходящие и неподходящие для донорства варианты анатомии.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. A safe removal of the liver right lobe and restoration of arterial blood supply to the liver graft is possible only with a full understanding of the anatomy of the hepatic artery in a donor.</p></sec><sec><title>Objective</title><p>Objective. To describe new and extend contemporary data on anatomical variations of the arterial blood flow in a donor of the right liver lobe.</p></sec><sec><title>Material and methods</title><p>Material and methods. From 2009 to 2021, 306 living donor liver transplantations were performed in the State Research Center – Burnasyan Federal Medical Biophysical Center of Federal Medical Biological Agency. The vascular anatomy of 518 potential donors was analyzed. Hepatic artery anatomical variants of a right lobe graft were assessed.</p></sec><sec><title>Results</title><p>Results. Eleven types of right lobe arterial supply and 7 subtypes of the arterial anatomy of liver segment 4 were identified. The case rates of types and subtypes where reconstruction could be performed were following: type A, subtypes 1, 2, 3, 4, 5 (57.5%, 26.1%, 5.5%, 1.9%, 0.3%, respectively); type B, subtypes 1, 4, 5 (0.3% each); type С, subtypes 1, 2 (2.9%, 1.3%, respectively); type D, subtypes 1, 3 (0.3% each); type Е subtype 1 (0.6%), types F-J subtype 1 (0.3% each). Liver right lobe harvesting and arterial reconstructions were fully performed in all types and subtypes excluding anatomical type K, subtype 7. Arterial postoperative complications (11 cases) were detected in 3.5% observed cases of 306 transplants and in 5.9% of all patients with complications (184). Mortality rate due to arterial complications was 1.9% (6 cases).</p></sec><sec><title>Conclusion</title><p>Conclusion. The existing classification of right liver graft hepatic artery anatomy was updated and detailed regarding the applicability in right lobe liver transplant.
