Оценить непосредственные и отдаленные результаты лигирования аневризм селезеночной артерии при ортотопической трансплантации печени у больных с декомпенсированным циррозом печени.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. С июня 2018 по май 2024 года в хирургической клинике ММНКЦ им. С.П. Боткина было выполнено 232 трансплантации печени, полученной от посмертного донора. У 4 пациентов (1,7%) на дооперационном этапе было выявлено наличие аневризматического расширения селезеночной артерии. Всем больным во время ортотопической трансплантации печени выполняли лигирование селезеночной артерии проксимальнее и дистальнее аневризм.</p></sec><sec><title>Результаты</title><p>Результаты. Среднее время этапа выделения и лигирования аневризм селезеночной артерии составило 18,4±3,3 минуты. Ни в одном случае не было повреждения и кровотечения из аневризмы селезеночной артерии, а также не было повреждения селезенки. Не было зафиксировано особенностей в раннем и позднем послеоперационном периодах, связанных с лигированием аневризм селезеночной артерии. Во всех случаях при контрольном обследовании ишемических изменений со стороны паренхимы селезенки выявлено не было.</p></sec><sec><title>Заключение</title><p>Заключение. Лигирование аневризмы селезеночной артерии во время ортотопической трансплантации печени – безопасное, эффективное и необходимое оперативное вмешательство, позволяющее улучшить отдаленные результаты лечения, снизив риск смертельного исхода от разрыва аневризмы в послеоперационном периоде.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective. The aim of our work was to evaluate the immediate and remote results of ligation of splenic artery aneurysm in orthotopic liver transplantation in patients with decompensated liver cirrhosis.</p></sec><sec><title>Material and methods</title><p>Material and methods. From June 2018 to May 2024, 232 liver transplants from a posthumous donor were performed at the Surgical Clinic of the Moscow Multidisciplinary Scientific and Clinical Center n.a. S.P. Botkin. In 4 patients (1.7%), the presence of aneurysmal dilatation of the splenic artery was revealed at the preoperative stage. During orthotopic liver transplantation, patients underwent ligation of the splenic artery proximal and distal to the aneurysms.</p></sec><sec><title>Results</title><p>Results. Mean time for the isolation and ligation of the splenic artery aneurysm was 18.4±3.3 minutes. In no case was there any damage or bleeding from either the branches or the aneurysm of the splenic artery; and there was no damage to the spleen. No adverse events related to the splenic artery aneurysm ligation were recorded in either early or late postoperative periods. In all cases, the control examination did not reveal any ischemic changes in the splenic parenchyma.</p></sec><sec><title>Conclusion</title><p>Conclusion.
