Инфекция области хирургического вмешательства, вызванная флорой контаминированного бактериями трансплантата почки, крайне редка и мало описана. Однако подобные случаи могут приводить к потере трансплантата и смерти реципиента, поэтому должны быть представлены профильным специалистам.</p></sec><sec><title>Цель</title><p>Цель. Демонстрация редкого случая развития инфекции области хирургического вмешательства у реципиента почки после пересадки ненамеренно инфицированного Klebsiella pneumonia трансплантата.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Реципиент почки – мужчина 49 лет, страдающий хронической болезнью почек 5-й стадии, которому выполнили трансплантацию почки от посмертного асистолического донора. При описании клинического случая были использованы результаты лабораторно-инструментальных и патоморфологических исследований из истории болезни, карты наблюдения реципиента и паспорта донора.</p></sec><sec><title>Результаты</title><p>Результаты. Представлена клиническая картина течения инфекции области хирургического вмешательства, вызванной бактериями K. pneumonia, которым был инфицирован трансплантат у реципиента почки, и результат ее лечения.</p></sec><sec><title>Выводы</title><p>Выводы. При трансплантации контаминированного микробами трансплантата почки развившуюся инфекцию необходимо считать глубокой. При прогрессивном развитии абсцессов, флегмон в ложе трансплантата и другой связанной локализации или течения инфекции по типу некротического целлюлита/фасциита при отсутствии эффекта от антибактериальной терапии необходимо выполнить срочное хирургического вмешательство с широким вскрытием очагов и удалением трансплантата почки как источника инфекционного процесса в сочетании с проведением адекватной антибактериальной терапии и отменой иммуносупрессии.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. A surgical site infection caused by bacterial flora of a bacteria-contaminated kidney graft is an exceedingly rare occurrence and is poorly described in the literature. However, such cases entail the risk of resulting in a graft loss and recipient death, and therefore should be attended to by a specialized team of experts.</p></sec><sec><title>Objective</title><p>Objective. This Case Report presents a rare instance of a surgical site infection in a kidney recipient following the transplantation of an unintentionally Klebsiella pneumoniae-infected graft.</p></sec><sec><title>Material and methods</title><p>Material and methods. The clinical case was a 49-year-old male suffering from stage 5 chronic kidney disease who underwent a kidney transplantation from a cadaveric asystolic donor. The clinical case was described using the results of laboratory tests, instrumental and pathomorphologic investigations obtained from the recipient medical history and observation chart, and the organ donor's passport.</p></sec><sec><title>Results</title><p>Results. This Case Report has described a clinical presentation of the surgical site infection with K. pneumoniae bacteria in a recipient of the infected kidney graft, and the treatment outcome.</p></sec><sec><title>Conclusions</title><p>Conclusions. In the event of infection developing in a bacteria-contaminated kidney graft, it is imperative to be suspicious of a deep infection. In the event of progressively developing abscesses, phlegmon in the graft bed and other related localizations, or the course of infection by the type of necrotizing cellulitis/fasciitis with no effect from antibacterial therapy, it is imperative to perform urgent surgical intervention with a wide opening of the foci and removal of the kidney graft as a source of the infectious process.
