Роль вируса простого герпеса в приживлении донорской роговицы

Аннотация

По данным Всемирной организации здравоохранения, в мире насчитывается 39 миллионов слепых людей. В развивающихся странах заболевания роговицы являются второй по распространенности причиной слепоты. Для множества слепых во всем мире трансплантация роговицы остается единственной возможностью вернуть утраченное зрение. Однако, согласно данным литературы, трупные донорские роговицы представляют потенциальную опасность передачи герпетической инфекции реципиенту при сквозной кератопластике. Известно, что персистенция вируса простого герпеса 1-го типа в донорской роговице способна негативно повлиять на приживление трансплантата вплоть до развития реакции тканевого отторжения. Реактивации латентного вируса простого герпеса способствует множество факторов, большинство из которых имеют место при сквозной кератопластике. Одним из таких факторов является иммуносупрессивная терапия – неотъемлемый элемент фармакологической защиты трансплантата. В случае перехода вируса простого герпеса в репликативную фазу в трансплантате роговицы целесообразно использовать противовирусные препараты. Наибольшее распространение в качестве противовирусных веществ получили препараты фармакологической группы интерферонов и индукторов интерферонов, а также аномальные нуклеозиды. Проведение герпесвирусной деконтаминации на этапе консервации позволит выполнять предоперационную профилактику передачи герпесвирусной инфекции от донора к реципиенту.
