ACTUAL ISSUES OF TRANSPLANTATION
The authors developed the method of skin grafting using a combined (epidermis-dermis) autograft that allows increasing the skin graft area two-fold or even more. We have assessed the survival of the dermal component of the graft in 10 patients who sustained full thickness burns of 5 - 15 %of the total body surface area (TBSA). We have demonstrated that the most complete survival of the dermal coinponent of the skin graft was observed in the cases of postoperative using the dressings containing epidermal growth factor (Hebermin ointment) and moisture- retaining film bandages (native-manufactured Breiemenderm film bandage). The period till the final epithelialization of the dermal component and donor wound made 2.5 - 3 weeks after grafting procedure. We hope that this period may be considerably shortened thanks to applying cellular technology and tissue engineering achievements in postoperative period. Improved results obtained in the survival of the dermal component of the combined skin graft allow using it to solve the problem of deficient donor skin resources in the patients with severe extensive burns.
Information got at the analysis of basic reanimation performance indicators of multifunction hospitals of Moscow Department of Health Protection included in the program of organ donorship (15 hospitals) for 7 years (2004-2010) is resulted. Findings testify to high donor potential, however reanimation material well-being of below existent requirement in the most of hospitals does not allow to provide the decline of level of insufficiency of donors.
PROBLEMATIC ASPECTS
The way of preparation and fractionating of blood from system of the bottom vena cava during operation with drawals of organs with mors of a brain is developed and approved. The new transfusion medium ~ a cellular component of blood of the donors, containing 1,7-5,4 standard doses of erythrocytes and 0,2-0,6 standard medical doses of thrombocytes is received. Biological full value and functional activity of cellular components of blood of donors-organs is shown. This cellular transfusion medium provides effective rising of oxygen-transport function of blood at an acute anaemia, moderate indemnification of a thrombocytopenia at liver transplantation.
Data on the efficiency of low-molecular peptide preparations, selected from cultivated stem cells of marrow, in the treatment of acute kidney insufficiency in experimental animals are resulted. It is demonstrated that the use of peptide preparations reliably promotes survivability of experimental animals suffering from acute kidney insufficiency.

DIAGNOSTICS
The basis of this work is the retrospective analysis of results of application of spiral computer and magnetic resonance tomography for 102 patients undergone to transplantation of abdominal and retroperitoneal organs in N.V. Sklifosovsky Institute. Possibility of early exposure of complications with the purpose of their timely correction is demonstrated.
The comparative analysis of two methods of exposure of sensitization of recipients to HLA is conducted - traditional serum method, based on a limphocytotoxicity test, and the method of multiplexed analysis with the detection of results using the Luminex platform. A high sensitiveness and informing of method of antibodies determination to HLA using the Luminex platform is demonstrated.
CASE REPORT
Utilization of kidneys donated from uncontrolled donors after cardiac death (UDCD) became an additional source of organs for transplantation worldwide in the light of ongoing global organ shortage. Machine perfusion proven to be a promising perspective for selection, rehabilitation and treatment of kidneys from expanded criteria donors, however, exact mechanisms of these effects remains unclear. On presented clinical case we are trying to clarify the essence of them. In 2006-2007 a pilot study was carried out by our team for use of machine perfusion to perform selection of kidney obtained from UDCD and improve their quality. According to study design each couple of grafts were divided and transplanted to different recipients. During ordinary procedure a staff mistake occurred, which, fortunately, helped us to understand one of the treatment mechanisms of hypothermic machine perfusion.
