Split-skin grafting is one of the most accepted and significant operations in the purulent surgery. In this study we appraised effectiveness and possibilitys of this method of dermepenthesis and discuss about speciality of split-skin grafting use in the purulent surgery.

Abstract

The efficacy and safety of everolimus in 10 recipients of liver in the post-transplant period with simultaneous reducing the dose of calcineurin inhibitors are analyzed. After conversion of immunosuppression the duration of monitoring of patients ranged from 4 to 18 months. Episodes of acute cellular rejection were not observed. Already in the first month after inclusion of everolimus in the scheme of immunosuppressive therapy, there was observed an improvement in renal function, which persisted at follow-up of patients. No serious adverse events were observed.

Abstract

The paper presents a 5-year experience of the Sklifosovsky Research Institute for Emergency Medicine in the treatment of patients after liver transplantation. The authors have reviewed the data on acute rejection in liver transplant recipients, analyzed the incidence of early liver transplant loss resulted from a non-controllable rejection, proposed prevention and treatment modalities for the cases of acute rejection.

Abstract

The study analyzes the reasons for the development of vascular complications, prevention and treatment after liver transplantation. In a retrospective study enrolled 206 patients who had undergone 214 orthotopic liver transplantation (OLT). Vascular complications has diagnosed in 18 (8.4 %) patients in postoperative period. A relatively low incidence of hepatic artery thrombosis (HAT) had seen in our study. Low molecular weight heparin (LMWH) plays an important role in the prevention of thrombosis. Doppler ultrasonography is a convenient and efficient method for detecting posttransplant complications and plays an important role in guiding treatment. Early diagnosis of these complications allows to decide on the tactics of the patients and to determine indications for emergency surgical revascularization and save the graft.

Abstract

The influence of postreperfusion syndrome on the results of orthotopic liver transplantation in 100 patients who underwent to liver transplantation from living related donors and 12 recipients who underwent to retransplantation of the liver was analyzed in the retrospective study. No statistically significant differences in the dynamics of lactate levels of specific enzymes, creatinine concentration at the third postoperative day, and duration of postoperative mechanical ventilation were detected between groups with and without the postreperfusion syndrome. In this retrospective observational study there was found no effect of postreperfusion syndrome on the functional state of the transplant and on the early postoperative course.

Abstract

The paper presents the up-to-date considerations on the role and place of the blood pumps of membrane type in the complex treatment of refractory heart failure. Physiological mechanisms involved in the myocardium morphological and functional recovery while using left ventricular bypass have been discussed. The paper shows that the development of a completely implantable artificial heart (AH) at the current stage of designing mechanical circulatory support devises is possible on the basis of membrane type blood pumps only.

Abstract

The results of multiple complex ultrasound examinations performed in 180 patients after the OLT in early and late postoperative period are analyzed. The presence of biliary anastomotic failure (BAF) after OLT was diagnosed in 6.1 % of recipients. Sensitivity, specificity and diagnostic accuracy of BAF by dynamic ultrasound observation in our study were 100, 98 and 99 % respectively. The emergence of biliary hypertension (BH) due to strictures of anastomotic and nonanastomotic localization was diagnosed by ultrasound in 12.2 % of operated patients. According to the study of dynamic ultrasound investigation previous ischemic damage to liver transplant or vascular complications were detected in 82.3 % of patients with the BAF and in 30.4 % patients with evidence of the presence of strictures of the biliary tree.

Abstract

We have previously found that the monitoring of the redox potential of the blood serum of patients after liver transplantation can detect acute graft dysfunction. In this study, investigations were performed according to a modified method of measuring the redox potential. The values of the redox potentials of healthy people were investigated to identify the normal levels of the measuring values of the potentials. Monitoring of the redox potential of the blood serum of patients with liver transplants gave us a possibility to establish the correlation between the magnitude and direction of shifts of potential and the development of complications such as cholestasis, pneumonia, septic conditions, an overdose of drugs. The relation between the values of redox potential shifts and the results of anti-inflammatory treatment has been detected.

Abstract

This paper describes the observation of 47 patients suffering from various kidney diseases and related metabolic syndrome. The evaluation of disorders of metabolic changes is carried out and a set of preventive measures including the use of non-drug means (not smoking, diet, physical rehabilitation), and drug therapy (statins, insulinotherapy if necessary) is proposed. Studies have shown that successful kidney transplantation in patients suffering from a chronic renal failure, coupled with the metabolic syndrome when using the complex measures aimed at the treatment of metabolic disorders can significantly improve the quality of life of patients.

Abstract

The paper describes the first experience with endovascular embolization of artery of kidney graft irreversibly lost its function in fifteen years after transplantation, during preoperative preparation of patient to nefrotransplantatectomy. The purpose of the study – to show the effectiveness of the artery rentgenendovascular embolization of the artery of kidney transplant before the nefrotransplantatektomy as a method of preventing massive intraoperative bleeding and related complications. An angiography and the rentgenendovascular embolization of the artery of kidney transplant executed on the day of surgery allowed us to remove nefrotransplantat without any technical difficulties. The total intraoperative blood loss did not exceed 50 ml.
