Abstract

The treatment outcome of 78 renal transplant recipients with a delayed graft function has been reviewed that accounts for 35.3 % of all operated patients. In the early postoperative period, 41 of them received a continuous venovenous hemodiafiltration, and 37 received a standard hemodialysis.

A continuous venovenous hemodiafiltration in the initial 24 hours after surgery appeared to make a more pronounced corrective effect on the renal graft function recovery. A later initiation of renal replacement therapy (RRT) was associated with a significantly longer oligoanuria period, and also with an increased incidence of acute rejection in the patients who received a standard hemodialysis.

Abstract

In this article we presented the new approach to the liver transplantation based on the prognosis paradigm of the tumor recurrence before the operation using largest experience of the liver transplantation in Russia. The effect independent clinicopathologic factors on outcome were tested using univariate and multivariate analyses by the Cox regression. The value molecular biomarker for choice of the patients to the transplantation liver was presented.

Abstract

In this report 43 donor cards of potential and actual non-heart-beating donors for 2008–2011 were retrospectively examined. It was noted that 15 donors did not match any of known categories in accordance with the Maastriht classification. In the analysis of this group of donors, it was found that all of them had clinical signs of brain death, but because of the impossibility of maintaining of «Instructions for stating a person's death on the basis of the diagnosis of brain death», a complete diagnosis of brain death, namely, performing the test of apnoetic oxygenation (separating test) was not possible. Removal of organs from these donors was carried out after the diagnosis of biological death only. The authors offer to supplement the Maastriht classification of non-heart-beating donors with a new category – the category VII (partially controlled donors).

Abstract

Chronic nephrotoxicity of calcineurin inhibitors (CNI-nephrotoxicity) is one of the most common causes of kidney graft dysfunction requiring correction of maintenance immunosuppressive therapy.

The aim of the study was to compare the efficacy and safety of the two possible approaches to dose adjustment of cyclosporine in patients with dysfunction of the transplanted kidney due to CNI-nephrotoxicity. It was established that the main cause of the abolition of Everolimus was proteinuria of nephrotic levels, developed in 19% of patients.

Abstract

We observed 44 patients during the first 2 – 3 weeks after liver transplantation. Depending on the postoperative course all patients were divided into 3 groups: control group consisted of 28 patients without complications, group 1 consisted of 9 patients diagnosed with an opportunistic viral infection; group 2 included 7 patients with the activation of viral hepatitis B or C in liver transplant . We analyzed the results of biochemical and immunological studies in all groups of patients. There were significant differences in changes in immunological parameters in the groups of patients with infection caused by viruses of different families.
