Abstract

Major surgical procedures induce a number of pathophysiological responses, which may lead to a significant increase in perioperative complications, delayed recovery and rehabilitation, and longer hospital stays. Fast-track surgery, also known as enhanced recovery after surgery (ERAS) or as multimodal surgery, aims at decreasing the frequency of postoperative complications, creating optimal conditions for earlier postoperative recovery and return to normal life activities.

The implementation of fast-track programs results in a reduced treatment duration without increase in readmissions. In this article, we have described our experience in the application of fast track program components to improve the outcomes after liver transplantation.

Abstract

The clinically introduced intraoperative morphological study of native kidney during transplantation of kindred kidney in patients with end-stage chronic renal failure, with intact water-excretory function before dialysis, and in the period of initiating the hemodialysis sessions significantly expanded our understanding of the pathogenesis, morphological characteristics of renal parenchyma diseases, and causes of their relapses in kidney transplant.

Abstract

Introduction. One of the priorities in oncology includes the development of alternative organoprotection treatments for renal cell carcinoma (RCC) of intraparenchymal and central tumor location.

Study objective was to develop the method of extracorporeal kidney resection without ureter transection in conditions of pharmacological cold ischemia, followed by an orthotopic autotransplantation for RCC.

Materials and methods. The study included 37 patients who had a morphologically confirmed RCC of stage pT1a-T3bN0M0-1G1-3 with intraparenchymal and central tumour location.

Results. The mean duration of surgery was 413.97 ± 89.14 minutes. The mean cold and warm ischemia time of kidney was 151.41 ± 41.29 minutes, and 8.39 ± 4.75 minutes, respectively. Intraoperative complications made 8.1% (3), postoperative complication rate was 48.6% (18).

Conclusion. Extracorporeal kidney resection without ureter transection in conditions of pharmacological cold ischemia, followed by an orthotopic autotransplantation in RCC allows the resection of large tumors in any location in conditions of prolonged cold ischemia.

Abstract

According to statistics, from 9 to 15 patients with electrical injury or electrical burn were annually admitted to the Burn Center of N.V. Sklifosovsky Research Institute for Emergency Medicine in the period from 2012 to 2015 that made about 1.5% of the total number of patients. The electrical injury, being a most severe and socially devastating injury, requires using combined therapies, applying the techniques of plastic and reconstructive microsurgery. The Burn Center specialists have developed up-to-date reconstructive techniques to quickly and efficiently treat tissue defects, including those after electrical burns.

Abstract

The article has summarized the global practice of the lung graft assessment with defining the absolute and relative criteria of donor lung suitability. The authors have presented their own experience of fiberoptic bronchoscopy 115 potential donors in the period from 2010 to 2015. The results of endoscopic examination of donor lungs were reviewed to determine the degree of the bronchial mucosa inflammation. A therapeutic fiberoptic bronchoscopy allowed clearing the bronchial lumen from pathologic contents and adequate preparing the lung graft in 29.6% of cases.
